Carrier Requirements

A quick, standard onboarding — same paperwork every serious carrier already has.

Documents We'll Need

Before we start booking loads, we'll collect the standard set of carrier documents required in the U.S. freight industry. Have them ready and we'll get you onboarded fast.

Active MC Number

Your Motor Carrier number, issued by the FMCSA, is your unique identifier as a freight carrier. It's essential for tracking your safety record and authority.

DOT Number

The Department of Transportation number is another key identifier for your business. It's used to track your inspections and compliance across state lines.

Certificate of Insurance (COI)

This document proves you have the necessary liability and cargo insurance to operate. We need this to ensure you and your loads are protected. Make sure to have Lloydie Logistics listed as a certificate holder.

W-9 Form

The W-9 form provides us with your taxpayer identification number. It's required for us to issue your 1099 form at the end of the year.

Signed Dispatch Agreement

This is our formal contract outlining the terms of our partnership, including commission rates and responsibilities. It protects both parties.

Notice of Assignment (NOA)

This document officially authorizes us, Lloydie Logistics, to act on your behalf to book loads and handle communications with brokers. It's a standard requirement in the industry.
